Saroj Naik, mom at 51 thru IVF

Saroj Naik's world collapsed in 2009 when her gynaecologist told her that she was menopausal and could never conceive. Although she had already had four miscarriages, 51-year-old Saroj and her husband Satyawan, 54, who reside at Merces, never lost hope and prayed for the gift of a child.

Their prayers seemed to have been answered when, through the method of in-vitro fertilization (IVF), Saroj was able to conceive again. The couple's elation knew no bounds when she ultimately gave birth to a healthy baby girl on February 10. Their joy doubled when they were informed that Saroj was the oldest woman in the state to have had a successful post-menopausal delivery with the help of IVF.

"Now, our days are filled with joy and happiness. Our day starts and ends with the hearty laughter of our little girl Saili," a proud Saroj said.

Fertility testing for a man is fairly straightforward, and involves testing the quality and quantity of semen, but there's more to it for a woman.

"For a man, it takes an afternoon," says Dr. Roger Pierson, a professor of obstetrics and gynecology at the University of Saskatchewan.

A couple of months ago, Daniels started looking at options. She came across several websites that talked about measuring Follicle Stimulating Hormone levels, or FSH, as a simple way of determining fertility. FSH is the hormone that promotes the growth and recruitment of eggs.

AMH assessment is also useful in fertility assessment as it provides a guide to ovarian reserve and identifies women that may need to consider either egg freezing or trying for a pregnancy sooner rather than later if their long-term future fertility is poor.Measuring AMH alone may be misleading as high levels occur in conditions like polycystic ovarian syndrome and therefore AMH levels should be considered in conjunction with a transvaginal scan of the ovaries to assess antral follicle count.

What the Numbers Won’t Tell You

Any woman who is struggling with fertility is likely to have dozens of numbers on her mind: bbt temps, sperm counts, follicle counts, uterine lining, estrogen, FSH, HCG, dosages of medications, age, numbers of cells, percents of morphology, success rates for clinics, success rates for procedures, days till ovulation, days till the next test... the list could go on and on.

Unfortunately, all too often these numbers become an identity for what a woman believes is her future. I cannot say how often I’ve heard a woman summarize her situation by saying something like:

“Well, my FSH is 30.”
“But my luteal phase is only 8 days long.”
“The numbers aren’t doubling.”
“I’ve got a 10% chance with my own eggs.”

These statements seem like simple facts, but they often signify so much more: I’m afraid, I’m too old, I’ve waited too long, There’s something wrong with me, There’s nothing I can do, I’m waiting to have a miscarriage, My heart is breaking, I don’t think I’ll ever be a mother. Somehow, in some clinical way, the numbers make it easier to say.

Infertility Causing Gene, Smad-3 Identified

Scientists at Virginia Commonwealth University have identified a gene, which they assert causes infertility. They said that, such types of genes send molecular signals used for the ovarian follicle development. This may be a guiding source for learning the fertility related issues in humans.

The scientists used a mouse model to examine the role of Smad-3 in the early stages of follicular growth.

They on careful observation found that, follicle-stimulating hormone, or FSH needs a particular gene to function in the body. So they confirmed that, the mice didn’t experience normal ovulation due to the absence of such gene.

Further, the researchers concluded that Smad-3 is an important class of proteins that are essential for follicle development.

"Learning precisely how the FSH receptor is regulated is an important step in understanding the subtle defects in signal transduction that can interfere with follicle development and female fertility and could lead to new types of fertility treatments," Prof Elizabeth McGee said.

These findings have been published in the current issue of 'Biology of Reproduction' journal.

Over 40 High FSH Success Stories

On the Women Over 40 With High FSH message board, there is a post called the Over 40 Timeline. It is a post that contains the successes of the women from that board. Whenever someone has a baby, they come back to add themselves to that post.

Many times though, the ladies are so busy after the birth that they don't have time to come back and post (which is completely understandable), so this isn't a comprehensive list.

But still, the number of success cases is amazing. It is a truly inspiring list. I refer to it often when my spirits are in need of a lift, like now!

Some interesting points:

* The oldest person on the list is Annie from Finland, with a baby at age 47, and highest FSH 37.
* Highest FSH on the list is Toni, who clocked in at 110.

RECOMMENDATIONS:
A2 Healthcare providers should not use random serum markers of follicle-stimulating hormone (FSH), luteinizing hormone (LH), and estradiol E2 for the purpose of predicting menopause since clear markers for predicting menopause are yet to be identified. (II-2)

I'm shocked because I went to the doctor and he did a blood test called FSH which showed I'm menopausal even though I'm having regular periods. How can this be?

Hormonal blood tests are notoriously unreliable as hormone levels vary widely from day to day and even during the day. The FSH test is basically useless for determining what stage of the perimenopausal transition anybody is in. Here are statements from the abstracts of three studies you can find on Medline. You might like to print out the whole abstracts and show them to your doctor if you want to convince him. Of course he *could* simply mean "perimenopausal" - the two words are often used interchangeably (and confusingly!)

It is concluded that FSH measurement is of little value, if any in the assessment of women during the menopausal transition because it cannot be interpreted reliably and because, apparently, ovulatory (and, presumably, potentially fertile) cycles may occur subsequent to the observation of postmenopausal FSH levels. Both oestradiol and inhibin are important negative feedback regulators of circulating FSH.

This discussion of the FSH test took me back to my menopausal transition, reminding me of some of the more horrifying absurdities foisted on me by the medicalization of menopause. For example, when, at 48, heavy bleeding drove me to the doctor's office, I was given the test - as part of a general check up to see what was going on, or so I thought. When the results came back the doctor told me that they would have to perform an endometrial biopsy to "check for cancer" since my FSH was "similar to a 29 year old's." Great.

So, then, after the (very painful for me) endo. biopsy showed nothing abnormal, they put me on ever-increasing doses of provera to curtail the bleeding - which paradoxically increased to the pointt where I had a 70 day continuous bleed accompanied by cramps just this side of childbirth. Weak from exhaustion/anemia, I dragged myself in again to the same doctor who then gave me a second FSH test. (The fact that I was taking 20 mg of provera a day seemed immaterial to him!)

Studying the new FSH results, he then told me that because of my unexplained POSTmenopausal bleeding I would require a more-or-less immediate hysterectomy. I should sign up right then and there because he was "very busy" in October.

When I reacted with shock, since the biopsy, which was normal, was also based on the FSH results which suggested a NON post-meno. status, the doctor became very condescending. He indicated that this second blood test indicated that my FSH had gone from 9 to 51 (in a couple of months) and suggested that I was now, suddenly, POST menopausal. Then he began to hint darkly at the possibility that I may have cancer or at least fibroids because POST menopausal bleeding is one of the major signals of serious trouble. As angry as it makes me to recount this, it also strikes me that the fact that the doctor was an insensitive jerk helped in the long run since my distrust prompted me to check out things on my own.

Fortunately, by this time, I'd discovered alt.support.menopause and was reading Susan Love's book as well as a number of books on hysterectomy, including Cutler's. Looking back and checking my "bleeding journal," the entire situation reminds me eerily of childbirth, in which case, the fetal monitor, like the FSH test, is often the excuse for rampant, unnecessary intervention in a normal physiological process....an intervention which may, in the end, have dire consequences (such as the loss of perfectly healthy body parts). Incidentally, this doctor's remark to me when I fired him to get a second opinion, was "don't come crying to me when you get ovarian cancer."

PS. To the newbies: I am doing just fine now. I feel better than I have since my 30s and have not bled for two years. I take no drugs other than the occasional ibuprofen and a multi-vitamin when I think about it. My only "symptoms" (hate that word) were a few hot flashes for several months as I made the adjustment to a drug free state. After what I went through, it was hard to go back to a doctor regularly though I do reluctantly get an annual PAP and mammogram.

A biologist at Wichita State University has made a breakthrough discovery about human reproductive hormones that scientists say could give women worldwide new hope in solving fertility problems.

Hormone Might Help Restore Female Fertility

TUESDAY, March 17 (HealthDay News) -- A hormone called kisspeptin may offer a new treatment for infertility, according to British researchers who found that the hormone can activate the release of sex hormones that control the menstrual cycle.

The study included 10 women who were not menstruating and were infertile due to a hormone imbalance. The researchers injected the participants with either kisspeptin or saline and then measured levels of two sex hormones -- luteinising hormone (LH) and follicle stimulating hormone (FSH) -- essential for ovulation and fertility.

The women who received kisspeptin had a 48-fold increase in LH and a 16-fold increase in FSH, compared to those who received the saline. The study is the first to show that kisspeptin can stimulate production of sex hormones in infertile women, according to the study's authors.

The findings were presented at a Society for Endocrinology meeting in the United Kingdom.

Researchers at the University of Texas Southwestern Medical Center in Dallas have discovered nearly 350 genes related to fertility, which may hold the key to causes of early infertility. “We hope to narrow down the genes with the greatest impact on fertility and test women to find out who carries defective versions; those who do can consider having children earlier,” says study author Diego Castrillon, M.D.

Override your DNA:

If any woman in your family has gone through menopause before age 40, or if you have erratic periods, ask your ob/gyn for a blood workup that includes a follicle-stimulating-hormone test; high FSH levels may indicate your ovaries aren’t functioning normally. Any woman can enhance her fertility by not smoking, limiting alcohol consumption and practicing safer sex. (STDs like chlamydia can damage reproductive organs.)

And if you’ve been trying to get pregnant for a year (six months if you’re older than age 35), ask for a referral to a fertility specialist: About 65 percent of women who seek medical assistance give birth successfully.

She is 47y old & so pregnant

There is a lady in my birthing unit that I work in, and she is being induced today. She is 47 years old, and so pregnant that she is being induced!!!

I am 29 weeks pregnant with my first baby. I turned 44 today.

I have had 3 miscarriages and my last FSH [follicle stimulating hormone] (tested over 2 years ago) was 18 on day 10. It was probably higher when we conceived this baby.

I was told by the first RE [reproductive endocrinologist] I saw, that my chances of conceiving were pretty slim (based on the theory that I would be a poor responder to treatment). Turns out I wasn't that great of a responder.

I got 9 eggs on my first IVF [in vitro fertilization], 8 on the second, 3 on the 3rd IVF and the last two cycles produced one egg each (both never fertilized).

I got pregnant once through IVF, but had a better success rate concieving naturally. Unfortunately, miscarriages were my issue.

The point of my long ramble, is that FSH is not an indicator of whether or not you will get pregnant or have a baby (especially conceived naturally), it's just a measure of how you MIGHT responds to the IVF meds.

I tell everyone I know not to give up, and to focus on trying natuarally if male factor and other female issues are not a problem. From my experience, IVF wasn't the best route to our baby, but thankfully I am beating the statistics and defying all the naysayers that told me a high FSH number equals never having a baby.

FSH:LH ratio significantly predicts IVF success, cancellation risk

Studying the value of the day 3 follicle stimulating hormone to luteinizing hormone ratio as a predictor of IVF success.

MedWire News: Measuring the ratio of follicle stimulating hormone (FSH) to luteinizing hormone (LH) on day 3 of an IVF cycle is an accurate way to predict cycle success and the risk for cancellation, report scientists.

FSH levels are used to predict chances of success but Ellen Greenblatt (Mount Sinai Hospital, Toronto, Ontario, Canada) and associates say they "are limited in value unless significantly abnormal."

For their study, the team retrospectively reviewed the value of the day 3 FSH:LH ratio for predicting treatment success. They studied 297 women aged under 40 years who had FSH levels of 10 IU/L or less and were undergoing IVF for the first time.

Analysis revealed that treatment was significantly more successful in women with an FSH/LH ratio of less than 2.0, with cycle cancellation rates of 8.5 percent versus 19.6 percent in those with a day 3 FSH:LH ratio of 2.0 or over.

Women with a low day 3 FSH:LH ratio were also more likely than other women to become pregnant, at rates of 33.5 percent and 24.7 percent, respectively. Indeed, no pregnancies occurred in women with a very high FSH:LH ratio of 3.5 or over.

"The day 3 FSH:LH ratio is a relatively easy test to obtain and adds more predictive power over the day 3 FSH level alone," the researchers conclude.

Analyzing the influence of age and basal levels of follicle stimulating hormone on the risk for aneuploidy in women undergoing IVF.

The risk of embryonic aneuploidy in women undergoing IVF is significantly influenced by age but not by follicle stimulating hormone (FSH) levels, say scientists.

Researchers from the Lister Fertility Clinic in London, UK, note the decline in fecundity seen in women of advanced maternal age “is attributed to reduction of ovarian reserve and deterioration of oocyte quality.

My interest was in comparing au natural success stories with ART. I ended up with three categories:
• Natural (no ART or produced the egg naturally and then did IUI),
• Natural but with Priming (had past ART or was in the process of ART, but conceived in a natural cycle. I view it as their system was primed by the ART)
