Time to update our Over 40 Birth Statistics Quote

It's time to update this quote:

"In 2007, there were more than 105,000 births in the United States to women ages 40 through 44, and over 7,349 to women 45 to 54. In 2007, there were 2,274 live births to women over 42, using donor eggs."

Checking the 2008 birth stats today at the CDC and I found:
All races and origins
40–44 years................. 106,090 live births
45–54 years................ 7,666 live births

TOTAL (40-54) : 113,756 live births

Next I went to SART, who has the 2008 IVF and Donor Egg rates in the US:

How old were the women who used ART in the United States in 2006?
Figure 3: The average age of women using ART services in 2006 was 36. The largest group of women using ART services were women younger than 35, representing 39% of all ART cycles carried out in 2006. Twenty-three percent of ART cycles were carried out among women aged 35–37, 19% among women aged 38–40, 10% among women aged 41–42, and 10% among women older than 42.

Number of live-birth deliveries to women using ART older than 42 in 2006: 4,134

How did the types of ART cycles used in the United States differ among women of different ages?
Figure 4 shows that, in 2006, the type of ART cycles varied by the woman’s age. The vast majority (96%) of women younger than 35 used their own eggs, whereas only 4% used donor eggs. In contrast, 21% of women aged 41 to 42 and more than half (55%) of women older than 42 used donor eggs. Across all age groups, more ART cycles using fresh eggs or embryos were performed than cycles using frozen embryos.

Here’s a wake-up call for anyone who thinks mature women don’t have to think about unwanted pregnancies.

Half of the pregnancies among women over 40 are unintended and 65 per cent result in abortion, said Kelty Moser, executive director of the Pictou County Centre for Sexual Health in New Glasgow.

Rising divorce rates mean more women in mid-life are encountering new sexual partners than in the past, and they aren’t accustomed to using condoms because the practice wasn’t taught when they went to school more than 20 years ago, Ms. Moser said.

Late motherhood boosts family lifespan

University of Utah demographer Ken R. Smith led a new study confirming that women who have their last baby after age 45 live longer than women who have their last...Same genes linked to longevity and births after age 45

SALT LAKE CITY, May 4, 2009 – Women who have babies naturally in their 40s or 50s tend to live longer than other women. Now, a new study shows their brothers also live longer, but the brothers' wives do not, suggesting the same genes prolong lifespan and female fertility, and may be more important than social and environmental factors.

"If women in your family give birth at older ages, you may well have a chance of living longer than you would otherwise," says the study's lead author, demographer Ken R. Smith, a professor of family and consumer studies at the University of Utah. "If you have a female relative who had children after age 45, then there may be some genetic benefit in your family that will enhance your longevity."

For descendants of the Utah and Quebec pioneers studied, "you may be able to look at the ages when your female ancestors gave birth – rather than just their longevity – in estimating how long you may live," says Smith, whose study will be published online May 4 and in the June 10 print issue of the Journal of Gerontology: Biological Sciences.

The researchers examined high-quality genealogical records from the Utah Population Database at the University of Utah with its records of 1.6 million Utah Mormon pioneers and their descendants. They also used the University of Montreal's Program on Demographic History Research, which has records on 400,000 people who lived in heavily Catholic Quebec between 1608 and 1850.

Specifically, the study involved the records of 11,604 Utah men who were born between 1800 and 1869 and who had at least one sister who lived at least to age 50; and the records of 6,206 Quebec men who lived between 1670 and 1750, and had at least one sister who lived to 50 or older.

It's time to update this quote:

"In 2005, there were more than 104,000 births in the United States to women ages 40 through 44, and over 6,500 to women 45 and older. In 2004, there were 1,786 live births to women over 42, using donor eggs."

Checking the 2007 birth stats today at the CDC and I found:

When I was a teenager, I thought you could get pregnant from sitting in a Jacuzzi. Not that I'm sure I even knew what a Jacuzzi was, but I knew they were racy places where racy things happened. Men, as far as I remember, didn't even have to be there. So long as they'd been in the water in some sort of excitable state, pregnancy was inevitable — even if you kept your pants on. You see, pregnancy was that easy, that dangerous, and it could ruin your life.

Almost 20 years later and pregnancy could still ruin my life. Although now, it is not the threat of what having a baby would do (no school, no education, no career, no future) but of not having a baby at all. I have been trying to get pregnant for two years and have not even come close to hearing the pitter-patter of tiny feet.

Analyzing the influence of age and basal levels of follicle stimulating hormone on the risk for aneuploidy in women undergoing IVF.

The risk of embryonic aneuploidy in women undergoing IVF is significantly influenced by age but not by follicle stimulating hormone (FSH) levels, say scientists.

Researchers from the Lister Fertility Clinic in London, UK, note the decline in fecundity seen in women of advanced maternal age “is attributed to reduction of ovarian reserve and deterioration of oocyte quality.

Age 'not a deciding factor' in motherhoodIt is lifestyle and health which are the deciding factors in whether to raise a child, it has been claimed.

Fit Pregnancy magazine stated that for many women, age is no longer an issue when it comes to choosing when to have a baby, but whether they have the strength, energy and health levels to cope is.

Mothers of varying ages spoke to the publication about their decision to have a baby earlier or later in life.

Shelley Promisel-Ryan explained that she delayed having her child until she was 43 because she wanted to give her career 21 years but then cut down her hours to spend time with her baby.

However, 27-year-old Heather Brown claimed she wanted to have her first child at 20 so that she can start her career when her son is 15.

The magazine concluded: "Neither the youthful 20s nor the wisdom refined from 40 years of life changes the rules - regardless of the joy that children bring, all mothers are tired and physically worn."

This is a list of clinics that I have found that are willing to treat over 50's, at least the ones who have replied to me.

I have researched the net extensively using my own criteria. I’ve researched in English and Spanish. My sisters in German and Swedish.

So we have covered quite a vast cross section of sites, blogs, articles and feedback.

And without doubt the Czech Republic came out on top. Especially for the Germans – who as you know don’t settle for anything less then the best.

I am sure there are other clinics and other counties that will suit different people's needs, but for me it will be Czech.

Wall Street Journal Examines Unplanned Pregnancy Among Older Women

Wall Street Journal columnist Johanna Bennett on Saturday examined unplanned pregnancies among older women in the U.S. and related health risks. According to Bennett, it is "not clear" how many women older than age 44 become pregnant because there is "little data" collected nationwide on pregnancy, abortion, miscarriages or contraceptive use among the group. CDC found that from 2000 to 2005, the number of live births among women ages 45 to 54 increased by 45% to 6,536, representing less than 1% of all live births in the U.S.

A survey conducted by the National Center for Health Statistics in 2001 -- the most recent data available -- found that about 40% of pregnancies among women ages 40 and older are unintended, and 56% of those pregnancies end in abortion. The survey also found that 7% of women between the ages of 40 and 44 had recently had sex without birth control but did not want to become pregnant.

Johns Hopkins Medicine found that women at age 45 have a 1% chance of conceiving using their own eggs, Bennett writes, adding, "Given such odds, it's no wonder many sexually active women over age 40 don't practice birth control." In addition, women with health problems have fewer choices in terms of contraception, according to Bennett. She notes that becoming pregnant and having an infant "later in life also comes with some increased health risks" for both the woman and infant. Bennett adds that compared to younger women, older women are more likely to have a miscarriage, experience problems during delivery, develop gestational diabetes and give birth to infants with genetic birth defects.

Vanessa Cullins, vice president of medical affairs for Planned Parenthood Federation of America, said, "It's very common that women don't realize they still need to worry about birth control even after they hit their 40's and move into their 50's." She added, "Until they complete menopause, which means going 12 months without menstruating, women should consider themselves to still be fertile" (Bennett, Wall Street Journal, 5/3).

Older women make grand mothers, say researchers

LONDON - Age is no bar to being a good mother and there is no reason, from the child's point of view, to stop pensioners becoming parents, researchers say.

Women in their 50's and 60's who conceive after fertility treatment are just as capable of being good parents as their sisters in their 30's and 40's.

The research will bring hope to thousands of women who have delayed parenthood and seek help late in life to have a family. Even though they run greater risks of complications during pregnancy and birth, once their children are born they are just as good at raising them.

The finding will also boost research into egg freezing, which could allow career women in their 20s to store their eggs for use in their 40's or 50's or even later. If egg freezing becomes a practical possibility for the average woman, it could fuel a boom in demand for fertility treatment from 40 and 50-year-olds.

Researchers from the University of Southern California studied the mental and physical health of 150 women, a third of whom had become parents in their 50s after in-vitro fertilisation with donor eggs. They found no significant difference in the health or stress levels of the older women that was likely to reduce their parenting capacity compared with younger groups in their 40s and 30s.

All the women gave birth between 1992 and 2004 at a single Los Angeles hospital and were sent questionnaires of which half were returned. On mental functioning, the over-50s were the only group who scored significantly higher than the national average.

The finding goes some way to answering critics of the trend to later motherhood who accuse women seeking fertility treatment in their 50's and 60's of being selfish and ignoring the welfare of their children.

But the study was criticised by a British specialist who said it followed women with children up to the age of 12 and did not show how older mothers would cope in their 60's and 70's when their children reached the more challenging teenage years.

Patricia Rashbrook, from Lewes, who became Britain's oldest mother at the age of 62 last July, defended herself against charges that she would be 80 before her child left school by saying it was the "quality of parenting that matters, not age".

The study from the University of Southern California supports her case.

Anne Steiner, who led the research, said: "The conclusion from this study, though it is limited and of small size, is that if we look from the perspective of stress and physical and mental functioning, it doesn't seem we can restrict parenting based on these reasons."

Is the Short Follicular Phase in Older Women Secondary to Advanced or Accelerated Dominant Follicle Development?

Abstract

This study sought to determine whether the shortened follicular phase in ovulatory older women is secondary to advanced (i.e. earlier) or accelerated (i.e. more rapid) folliculogenesis. Normal ovulatory women, aged 40–45 yr (n = 15) and 20–25 yr (n = 13), underwent daily venipuncture and transvaginal ultrasonography throughout the follicular phase of a spontaneous menstrual cycle (control cycle) and after pituitary down-regulation with a GnRH agonist (study cycle).


As expected, the older subjects in the control cycles demonstrated an elevated day 3 FSH and a shortened follicular phase compared with the younger subjects. After release from hypothalamic-pituitary-ovarian axis suppression, the early follicular phase FSH peak occurred earlier (6.8 vs. 9.8 d; P greater than 0.01) and was of a greater magnitude (12.1 vs. 6.5 mIU/ml; P greater than 0.01) in the older subjects.

The time from release of suppression until the subsequent LH surge was also shorter (17.5 vs. 20.8 d; P greater than 0.01) in the older group. However, the time from FSH peak to LH surge was similar in the older and younger groups (10.7 vs. 11.0 d; P = 0.74). Compared with younger women, older subjects had normal follicular phase levels of estradiol and inhibin A and lower levels of inhibin B in both control and study cycles.

We conclude that the shortened follicular phase observed in older ovulatory women is due to earlier dominant follicle selection, independent of hormonal influences from the preceding luteal phase.

Despite physical toll, more over-40 women having babies

Kim Cabral of Brewster thought she was in early menopause. Instead, she found out she was pregnant.

In April 2006, Cabral gave birth to her third child, William, at age 45. During the pregnancy, she was "shocked and scared," so convinced was she that at her age the baby would be born with birth defects. She also discovered that childbearing is a different story after 40.

"I was preparing myself for (birth defects) by talking to friends and doing research," says Cabral. "I can tell you that having a child in my 20s, 30s and 40s had extreme differences each time."

Cabral is hardly alone in learning that lesson. On March 16, Oscar-winning actress Halle Berry, 41, gave birth to her first child, a girl. The number of U.S.-born women ages 40 to 44 giving birth has tripled since 1982, according to a recently published report from the Public Policy Institute of California. The numbers rose during that period from 3.5 to 10.5 births per 1,000 women.

Sally Goldberg, a parenting specialist and author of "Constructive Parenting" (Allyn & Bacon, 2001), points to an "increasingly complex society" for those escalating figures, with women getting married later and waiting longer to have children.

"A major reason for waiting longer is getting their careers under way. Another is the rise in infertility," she says. "It takes a while to determine that infertility is a factor and then even more time to treat it. A third reason is finances. It costs a lot to raise a child, and many couples need the time to save up for the responsibility."

But women have so many responsibilities these days that having young children can be especially stressful for women in their 40s.

Besides full-time jobs outside the home, Goldberg says, "Moms also have the mom-full-time job — basically 40 hours of work plus the additional 24/7 responsibility." Many moms are also students, full time or part time, plus "we all know it takes hours of work to take care of a home, and hours of work to love, honor and cherish a mate." Mom care — the hairdresser, clothes shopping, doctor visits, etc. — "all take time, so the list goes on."

Different decades

In her 20s, Cabral had had tons of energy, bodysurfing waves off Wellfleet the day her water broke with the first baby. And it was a good thing she had that energy.

"My son Rob was and still is my most active child. He could do a running flip from the ground at 3," she says. "I was also very socially active and took Rob to a lot of (political) demonstrations."

While carrying her second child at age 33, Cabral was "thrilled." As director of Kit Anderson House shelter in Hyannis, she worked until three days before her due date.

"I gave Rob a birthday party at an arcade — and that night, delivered Autumn. She was born in less than three hours, and Rob and Autumn have the same birth date! But I was so exhausted being a working mom, I didn't get to enjoy her babyhood like I wanted."

Time went on, and Cabral became a preschool teacher. Life changed when she started feeling strange.

"I figured out I was pregnant with William by the second month because my period hadn't come and I was tired and dizzy all the time," she says. "With William, it was entirely different from the beginning, mostly from the fatigue factor and I had morning sickness from the second month throughout the rest of the pregnancy. ... Then I had an ultrasound — and there he was — it looked like his hand was waving."

Cabral's doctors immediately let her know she had beaten the odds: There is only a 7 percent chance that a woman in her 40's can conceive naturally. Most women that age conceive through artificial insemination or donor eggs. There were also warnings.

"The OB-GYN laid it down for me," she explains. "At this age there was a huge risk factor. Down syndrome is the biggest risk for women in their 40s." In addition, the risk of miscarriage increases with age.

So it should come as no shock that this most recent pregnancy was hard for Cabral.

"I left my job two months early. I was so exhausted I just knew I couldn't do both."

Benefits, too

Being an at-home mom has been a different experience. Cabral acknowledges, "I felt isolated at first, but as William grew, we joined baby yoga and play groups. It's true I don't meet many women my age that are mothers, but that's OK. I have to also say this time around is very tiring, but I love every minute."

Stacey Horton, 43, of South Yarmouth is the mom of Will, 6, and Olivia, 3. She agrees with Cabral.

"Universally, our children are a gift," she says. "They bring us joy and teach us our most valuable lessons. They allow us to stay youthful and express our heart's desires."

There are definite benefits to having a baby later in life. The chief one that Cindy Horgan, family support coordinator at Cape Cod Children's Place in Eastham and co-creator of its parenting station, sees is that women over 40 "have a good sense of who they are and tend to be focused on guiding a young child because they aren't focusing on their own identities."

But, like Cabral, Horton feels having young children later in life can be challenging:

"My energy level is not what it used to be, and my stress level has begun to soar. Although our children get along very well, there are typical sibling rivalry issues. These can be extremely challenging, especially being someone who doesn't handle conflict very well. Again, a lesson for me."

Since Will had been born three months prematurely, her second birth was meticulously monitored.

Fortunately, Horton says, "Other than the typical three months of nausea, my pregnancy was fairly easy. Our daughter was born three weeks early, which was perfect for me."

Advances in medicine that allow that monitoring have helped more women conceive after age 40 and stay healthy during those later-in-life pregnancies. While there are more risks for that age group now, the risks were even greater a generation or two ago.

Taking a chance

One mother of six — who did not want her name used, but is affectionately called "Mama Jean" by friends and family — had her sixth child at age 41, in 1964.

"I was lucky to have six kids without any problems back then, especially since my blood was RH negative, which could cause health problems to the baby," she says. "I only stopped having children at 41 because the doctor told me I'd be taking a chance at my age to have more."

Back then, Mama Jean shares, "I would have kept having children if I'd been younger."

But nowadays, with priority on education and career before kids, women are willing to wait to give birth. And Cabral offers encouraging words:

"Doctors use new ultrasounds now that trace every physical aspect of the baby to determine the baby's health. So they know early on of any serious developmental and/or physical delays."

Using these new techniques, Cabral was given an ultrasound when she was less than four months pregnant, and then another when she was seven months along. She went through a variety of other medical monitoring, including a stress test, and doctors finally decided to induce labor to encourage the birth. William was a healthy, 9.4-pound baby.

More risks, more stress, more physical challenges — yet over-40 moms say the prize of the tiny life is worth it.

"My daughter has brought me such joy from the moment she was born," Horton says. "Although having one child would have been more manageable, I can't imagine our lives without my daughter. She is a beacon of light and a great sibling."

Cabral agrees. She marvels: "Each child is special, but when you're older, you cherish each little thing. My husband and I were at the playground the other day and he said, 'What would we be doing now without William?' I answered, 'We'd be home watching TV.'"
