We have all heard stories about friends coming back from a wine-soaked holiday to discover, to their astonishment, that they are pregnant. There’s a palpable sense of how did that happen? The reality is that, for most women struggling with fertility problems, this is the exception rather than the rule. However, research does suggest that taking care of what we put in our bodies may have a profound effect on upping our chances of getting a positive pregnancy test, even to the point of being as effective as IVF treatment.

In a review of the recent research, Dr Emma Derbyshire and her team from Manchester Metropolitan University revealed that in one study, while intrauterine insemination and ovarian stimulation gave a success rate of 33 per cent in conception, dietary advice was virtually as effective, leading to a 32 per cent success rate.

Budgeting for InfertilityOvulation induction: Medication is administered to cause the release of more eggs at one time. Typically, when fertility difficulties are suspected, this is a first approach.
Cost: $100 to about $3,000 or more.

Intrauterine insemination (IUI): Semen is collected and inserted through the vagina and cervix into the uterus with a catheter. Often combined with ovulation induction.
Cost: $1,000 to $2,000.

In-vitro fertilization (IVF): Eggs are harvested from the mother; then eggs and sperm from a partner are combined in a lab. The fertilized egg is placed back into the woman's body to develop. This can be done using a donated egg or donated sperm.
Cost: At least $10,000 to $15,000.


Gamete intrafallopian tube transfer (GIFT): After ovulation, eggs are harvested from the mother. Sperm and egg are placed beside one another in a woman's fallopian tube in hopes that fertilization will occur.
Cost: $15,000 to $20,000.


Embryo adoption: A fertilized egg from one couple is "adopted" by another couple and then implanted for development.
Cost: $6,000 to $18,000.

So when month after month of "TTC" (trying to conceive) goes by and "2WW"(two weeks waiting) gets you a "BFN" (big fat negative) on your home pregnancy test, you and your "DH"(dear husband) are understandably disappointed, frustrated and shocked. Eventually many couples seek help, have a fertility workup and get prescriptions for injectable fertility drugs as part of a treatment plan which may also include IUI (intrauterine insemination) or IVF (in vitro fertilization).

My interest was in comparing au natural success stories with ART. I ended up with three categories:

• Natural (no ART or produced the egg naturally and then did IUI),

Clomid, IUI no help for some types of infertilityA new study suggests that using the medication Clomid and undergoing intrauterine insemination (IUI) offer no benefit to women who suffer from unexplained infertility.

Researchers point out that many doctors use both of these methods on women who show no obvious reason for their infertility because "doing nothing" is not popular.

But in a study of 580 women, those who used Clomid or underwent IUI were no more likely to get pregnant than if they had done nothing. Experts point out that both Clomid and IUI are highly useful, but only if women have trouble ovulating or if there is a sperm motility issue.

Background
There are different factors that influence treatment outcome after ovarian stimulation and timed-intercourse or intrauterine insemination (IUI). After patient age, it has been suggested that timing of insemination in relation to ovulation is probably the most important variable affecting the success of treatment. The objective of this study is to study the value of human chorionic gonadotropin (hCG) administration and occurrence of luteinizing hormone (LH) surge in timing insemination on the treatment outcome after follicular monitoring with timed-intercourse or intrauterine insemination, with or without ovarian stimulation.


Methods
Retrospective analysis of 2000 consecutive completed treatment cycles (637 timed-intercourse and 1363 intrauterine insemination cycles). Stimulation protocols included clomiphene alone or with FSH injection, letrozole (an aromatase inhibitor) alone or with FSH, and FSH alone. LH-surge was defined as an increase in LH level ≥200% over mean of preceding two days. When given, hCG was administered at a dose of 10,000 IU. The main outcome was clinical pregnancy rate per cycle.


Results
Higher pregnancy rates occurred in cycles in which hCG was given. Occurrence of an LH-surge was associated with a higher pregnancy rate with clomiphene treatment, but a lower pregnancy rate with FSH treatment.

Conclusions
hCG administration is associated with a favorable outcome during ovarian stimulation. Awaiting occurrence of LH-surge is associated with a better outcome with CC but not with FSH treatment.

They say it's actually the core of the pineapple that works best, not the pineapple itself. It's an enzyme called Bromelain, you can also take tablets from the health food store, but I don't know if it would work as well. Some say it needs to be fresh pineapple, because it is an enzyme.

However, some say that fresh pineapple, if eaten in excess could cause a miscarriage, as it can cause uterine cramping. So if you are going to try it, it might be better to stick to the following routine:

Using Pineapple to Assist Implantation: Pineapple contains bromelain. Bromelain is a proteolytic enzyme that breaks up proteins that inhibit embryo implantation. Consuming a whole pineapple (focusing on the core as it has the highest concentration of Bromelain) during a fertility cycle can be beneficial.

For IVF [in vitro fertilization] cycle: one pineapple, divided into 5 portions - consume one portion each day, over five days, beginning on the day of your embryo transfer.

IUI [intra-uterine insemination] cycle: one pineapple, divided into 5 portions - consume one portion each day, over five days, beginning on the day of your IUI.

Timed intercourse cycle: one pineapple, divided into 5 portions - consume one portion each day, over five days, beginning on the day after ovulation.

Note: Be careful not to start consuming pineapple too early in the cycle. Beginning to consume pineapple BEFORE the recommended days above can actually affect your vaginal and cervical mucus PH, making it more acidic.

According to the Center for Disease Control's (CDC) 2002 National Survey on Family Growth, 1.2 million of the estimated 62 million women in their reproductive years – that's two percent – have had infertility-related medical appointments; 10% have received infertility services.


The American Society of Reproductive Medicine (ASRM) reports that most infertility cases – 85%-90%, in fact – are treated with conventional medical therapies (medication or surgery.) But for some, in vitro fertilization and other Assisted Reproductive Technologies (ART) are needed to try and conceive.

Intrauterine insemination (IUI)

How does it work: Intrauterine insemination, also known as artificial insemination, involves a woman being injected with washed and treated sperm from her husband or a donor during ovulation. During this time, she must also take fertility drugs to stimulate egg production. Couples may try IUI if there is a problem with low sperm count or mobility, or another unexplained infertility condition.

How much does it cost? Costs associated with IUI's vary, as with other procedures, and are dependent on how many cycles a woman goes through and whether the sperm is donated, as well as additional costs associated with medication, ultrasounds and monitoring. Ranges for IUI cycles can be anywhere from the mid $300's to the $1000's, and, as with all infertility treatments, eligibility for insurance coverage depends on where you live, the clinic you are visiting and your insurance carrier.

In vitro fertilization (IVF)

How does it work? In vitro fertilization takes place in a lab rather than in a woman's fallopian tubes. Eggs are surgically removed and fertilized with sperm in a dish before a fertilized embryo is transferred into the uterus. According to the CDC, this is the most widely used ART procedure, accounting for 90% of ART cycles in the US.

How much does it cost? One cycle of IVF can cost well over $6,000, with many women having to undergo more than one cycle for success.

Gamete intrafallopian transfer (GIFT)

How does it work? Gamete intrafallopian transfer requires the removal of eggs and sperm and placing them directly into the fallopian tube. Rather than fertilization occurring in the lab, GIFT takes place inside the fallopian tubes.

How much does it cost? Estimates for GIFT procedures run in the double digits, with an average cost of more than $12,000.

Zygote intrafallopian transfer (ZIFT)

How does it work? Zygote intrafallopian transfer involves fertilizing a woman's egg in a dish and surgically transferring that egg into the fallopian tubes.

How much does it cost? ZIFT procedures can cost more than other ART procedures with an average range of $15,000+ per cycle.
